Jordan Yates 6f41d52734 kernel: switch to `SYS_INIT_NAMED`
Update the two locations that use two `SYS_INIT` macros with the same
initilisation functions to use `SYS_INIT_NAMED`.

Gerard Marull-Paretas cffefc818d kernel: migrate includes to <zephyr/...>
In order to bring consistency in-tree, migrate all kernel code to the
new prefix <zephyr/...>. Note that the conversion has been scripted,
refer to zephyrproject-rtos#45388 for more details.

Daniel Leung 5c4fff3998 kernel: kheap: make init work with demand paging
With demand paging, the heap object and its backing memory
may not be in physical memory. So initialize those heaps
in pinned region at PRE_KERNEL_1 and the remaining heaps
once paging mechanism has been initialized.

Andy Ross 3c2c1d85b0 kernel: Remove z_mem_pool wrapper internals
These implemented a k_mem_pool in terms of the now universal k_heap
utility.  That's no longer necessary now that the k_mem_pool API has
been removed.

internal usage now uses equivalent "z_mem_pool" symbols instead.

Andy Ross 8a6aee9cac kernel: Make the "heap" backend to mem_pool default
Remove the MEM_POOL_HEAP_BACKEND kconfig, treating it as true always.
Now the legacy mem_pool cannot be enabled and all usage uses the
k_heap/sys_heap backend.

Tomasz Bursztyka e18fcbba5a device: Const-ify all device driver instance pointers
Now that device_api attribute is unmodified at runtime, as well as all
the other attributes, it is possible to switch all device driver
instance to be constant.

Andy Ross 0dd83b8c2e kernel: Add k_heap synchronized memory allocator
This adds a k_heap data structure, a synchronized wrapper around a
sys_heap memory allocator.  As of this patch, it is an alternative
implementation to k_mem_pool() with somewhat better efficiency and
performance and more conventional (and convenient) behavior.

Note that commit involves some header motion to break dependencies.
The declaration for struct k_spinlock moves to kernel_structs.h, and a
bunch of includes were trimmed.
