Jiri Slaby 37818afd15 x86/asm: Do not annotate functions with GLOBAL
GLOBAL is an x86's custom macro and is going to die very soon. It was
meant for global symbols, but here, it was used for functions. Instead,
use the new macros SYM_FUNC_START* and SYM_CODE_START* (depending on the
type of the function) which are dedicated to global functions. And since
they both require a closing by SYM_*_END, do that here too.

startup_64, which does not use GLOBAL but uses .globl explicitly, is
converted too.

"No alignments" are preserved.

Jiri Slaby 28be1b454c x86/boot: Remove unused copy_*_gs() functions
copy_from_gs() and copy_to_gs() are unused in the boot code. They have
actually never been used -- they were always commented out since their
addition in 2007:

  5be8656615 ("String-handling functions for the new x86 setup code.")

So remove them -- they can be restored from history if needed.

David Woodhouse e24df921af x86, boot: Use .code16 instead of .code16gcc
This reverts commit 28b48688 ("x86, boot: use .code16gcc instead
of .code16").

Versions of binutils older than 2.16 are already not working, so this
workaround is no longer necessary either.  At the same time, some of
the transformations that .code16gcc does can be *extremely*
counterintuitive to a human programmer.
